We provide transitional housing and crisis respite for children and adults, offering a safe and stable home environment during times of transition or heightened need. With 24/7 staffing and consistent program oversight, our team ensures safety, structure, and steady guidance.
Our services include individualized care planning, behavioral and positive support services, transportation assistance, and community integration to build confidence and life skills. We support residents with daily living activities such as hygiene routines, meal preparation, organization, and home care skills. Lumos also offers medical administration assistance and encourages participation in meaningful daily activities to promote emotional well-being, independence, and connection.

Our MDH-licensed homes are located in Cannon Falls, Red Wing, and Apple Valley—each designed to feel warm, safe, and truly home-like. Lumos residences offer comfortable bedrooms, welcoming living spaces, and nurturing environments supported by trained staff available 24/7.
Each location provides structure, stability, and a family-like atmosphere for children and adults in need of transitional housing or crisis respite. Our homes are situated in safe, quiet neighborhoods with access to community resources, parks, schools, and essential services.
